Submit order forms, insurance cards, and other scanned documents
Document Upload Guidelines:
- File Types Accepted: PDF, PNG, JPG, or JPEG formats
- Document Types: Order forms and/or insurance cards (front and back)
- Multiple Files: You can upload multiple documents at once
- File Size Limit: Total size must not exceed 20 MB
Ensure all documents are clear and readable before uploading.
